Responsibilities:
- Obtain temperature, pulse, blood pressure, and respiration
- Maintain a clean, safe, and healthy environment
- Assist patient with getting in and out of bed, bathing, hygiene, and toileting
- Prepare and provide and/or assist with meals
- Perform light homemaking and other environmental services
- Observe, report, and document any changes in patient status
- Understand basic elements of body functioning and report changes in patient’s body functions as indicated
- Recognize emergency situations and implement appropriate emergency procedures per care center policy
- Follow care center procedures regarding infection prevention and control, handling of hazardous waste, and safety measures
- Follow specific written instructions for personal care in accordance with the plan of care
- Assist patient with prescribed exercises as instructed per nurse or therapist
- May involve respite support, providing short-term care and supervision to relieve family caregivers
- Perform other duties as assigned
Qualifications:
- High school diploma or equivalent
- Current state certification (or registration) as a nurse assistant as per state certification/registration requirements
- CNA experience within the last two years
- If it has been more than two years, you must complete another training and competency program before providing services
- Must be able to successfully pass the competency program of the care center
- Current CPR certification
Responsibilities:
- Designing and implementing daily activities and programs that meet the social, emotional, and physical needs of clients
- Coordinating schedules, staffing, and resources to ensure smooth program operations
- Collaborating with caregivers, nurses, and other team members to integrate activities with individual care plans
- Monitoring client participation and progress, and adjusting programs as needed
- Ensuring compliance with agency policies, licensing requirements, and health and safety standards
- Maintaining accurate documentation of activities, attendance, and program outcomes
- Communicating regularly with families and guardians to share updates and encourage involvement
Responsibilities:
- Perform patient assessments and collaborate with the care team to develop and implement a plan of care
- Make referrals to other disciplines as indicated by the patient’s identified needs, or document rationale for not doing so
- Promote patient health and independence through teaching and appropriate rehabilitative measures, assisting patients in learning self-care techniques
- Supervise LPNs and HHAs
- Complete documentation timely and accurately
- Regularly communicate patient progress to the clinical manager and care team
- Plan and provide staff education
- Perform on-call responsibilities and on-call services to patients/families as assigned
- Participate in clinical development and continuing education programs
- Perform other duties as assigned
Qualifications:
- One (1+) year of clinical experience as a Registered Nurse (RN)
- If less than 1 year clinical experience as an RN, candidate must be approved by VP Clinical
- Current RN license, specific to the state(s) you are assigned to work
- Current CPR certification
- Valid driver’s license, reliable transportation, and liability insurance
- Note: If less than 6 months clinical experience as an RN, candidate must participate in RN Intern program
Responsibilities:
- Verify plan of care with the RN prior to each visit
- Collaborate with physician and RN frequently to ensure patient needs are being met; notify them as soon as possible when patient condition changes
- Teach patients and caregivers self-care techniques
- Check vital signs
- Review all medications and perform a medication reconciliation at each visit
- Perform specialized procedures ordered by physician (i.e. foley catheter change)
- Prepare equipment and materials for treatments using the aseptic technique
- Complete documentation timely and accurately
Qualifications:
- One year of clinical LPN experience
- Current LPN license, specific to the state(s) you are assigned to work
- Current CPR certification
- Valid driver’s license, reliable transportation, and liability insurance
Responsibilities:
- Provide physical therapy services and interventions under the direction and supervision of a physical therapist and in accordance with the physician-approved plan of care
- Observe, record, and report the patient’s response to treatment and any change in condition to the supervising PT, physician, and other appropriate clinical staff
- Provide resources for future needs and promote wellness and prevention
- Include documentation of supervisory activities in the patient’s medical record
- Monitor and document the patient’s response to physical therapy intervention
- Participate in patient care conferences, in-services, chart audit activities, quality improvement activities including OBQI and OBQM, and peer-review activities
- Maintain confidentiality of information relating to the physical therapist-client relationship
- Make recommendations for follow-up care in coordination with the agency that admitted the patient, and utilize other agency care providers to assist with referrals to community resources
- Perform other duties as assigned
Qualifications:
- Current license to practice as a physical therapy assistant, specific to the state you’re assigned to work in
- Graduate from an accredited physical therapy assistant educational program as set by the Commission on Accreditation of Physical Therapy Education (CAPTE)
- One year of related physical therapy experience for PTAs entering home health care to ensure knowledge and skills are sufficient to safely provide services to patients
- If less than one year of experience, approval from regional clinical leaders is required as well as appropriate mentoring and/or participation in a residency program
- Current CPR certification
Responsibilities:
- Evaluate patients and develop individualized treatment plans to improve or restore functional daily living skills
- Implement therapy programs and instruct patients in the use of adaptive equipment to improve independence
- Provide training and education to family members and caregivers on patient care techniques and strategies
- Collaborate with physicians, nurses, and other healthcare team members to coordinate patient care
- Monitor patient progress and adjust treatment plans as necessary
- Complete timely and accurate documentation of patient care, evaluations, and progress notes
- Participate in interdisciplinary team meetings and discharge planning
- Maintain confidentiality of patient information in accordance with HIPAA and company policies
- Perform other duties as assigned
Qualifications:
- Current license to practice as an Occupational Therapist, specific to the state you are assigned to work
- Graduate from an accredited Occupational Therapy program
- Current CPR certification
- Valid driver’s license, reliable transportation, and liability insurance
- One (1+) year of clinical experience as an Occupational Therapist preferred
- Strong communication, organizational, and critical thinking skills
Responsibilities:
- Work to prevent, assess, diagnose, and treat speech, language, social communication, cognitive communication, and swallowing disorders in adults
- Provide aural rehabilitation for individuals who are deaf or hard of hearing
- Provide augmentative and alternative communication (AAC) systems for individuals with severe expressive and/or language comprehension disorders
- Perform other duties as assigned
Qualifications:
- Current, unencumbered license to practice speech pathology specific to the state you are assigned to work
- Current Certificate of Clinical Competence in Speech-Language Pathology (CCC-SLP)* with completed graduate coursework and a clinical practicum at a college or university whose program is accredited by the Council on Academic Accreditation in Audiology and Speech-Language Pathology (CAA)
- Current CPR certification
Preferred:
- One (1+) year of experience in speech pathology
Responsibilities:
- Assist patients with daily living activities, including bathing, grooming, dressing, toileting, and feeding
- Measure and record vital signs such as temperature, pulse, blood pressure, and respiration
- Help patients with mobility and transfers, including getting in and out of bed or chairs
- Maintain a clean, safe, and healthy environment for patients
- Observe and report any changes in patient condition to the supervising nurse
- Provide emotional support and companionship to patients
- Assist with range of motion exercises and other therapeutic activities as instructed
- Follow care plans and specific instructions provided by nursing staff
- Maintain patient confidentiality and adhere to HIPAA guidelines
- Perform other duties as assigned
Qualifications:
- High school diploma or equivalent
- Current STNA certification (State Tested Nursing Assistant)
- CNA or STNA experience within the last two years
- If it has been more than two years, completion of a new training and competency program is required before providing services
- Ability to successfully pass the care center’s competency program
- Current CPR certification
- Valid driver’s license, reliable transportation, and liability insurance
